This class will be held on Mondays, Wednesdays, and Fridays from 9am-11:30am. Tuition is $135 per month, with a $60.00 registration fee* due today. This class is for students 4-5 years old (Kindergarten next school year) and potty trained.

This class will be held on Tuesdays and Thursdays from 9am-11am. Tuition is $110 per month, with a $60 registration fee* due today. This class is for students 3-4 years old and potty trained.

Release Waiver

The contract tuition period is nine months. Monthly tuition is paid in nine equal monthly payments due on the 1st of each month--prior to services being rendered. I understand and agree to pay an automatic late fee of $25.00 if paid after the 5th of any month. I understand that there will also be a $25.00 returned check fee. Please keep us updated on any changes to your account. We do not prorate tuition for vacations or extended absences from school. We will follow the Box Elder School District schedule as close as possible.
I agree that this waiver and release of liability shall apply to each day my student is at Happy Hive Preschool, located at 146 S 200 W, Brigham City regardless of the date this form is signed below. Happy Hive Preschool will not be held responsible for any and all injuries, losses, or damages that might occur while on the premises of the preschool, as well as its playgrounds and facilities, and also any offsite preschool program or activity: and to the maximum extent of the law, I agree to waive and release any and all claims, suits, or related causes of action Happy Hive Preschool, their owners, officers, employees or agents for injury, loss, death, costs or other damages to me, my heirs or assigns, or third parties for claims, suites or related cause of action asserted against the preschool owners or owners of the property of 146 S 200 W, Brigham City arising from my conduct and/ or my family’s conduct while participating in the preschools programs or activities. I further agree to release, indemnify, defend, and hold Happy Hive Preschool harmless from any liability whatsoever for future claims presented by my child for any injuries, losses, or damages.
Covid-19:
By signing the below, I also agree to follow the policies and procedures put into place by Happy Hive Preschool, LLC regarding Covid-19. I further agree to release, indemnify, defend, and hold Happy Hive Preschool, LLC harmless from any liability whatsoever for future claims presented by my child for any injuries, losses, or damages due to Covid 19.
By signing below, I acknowledge that my child’s picture may be posted on social media and/or other advertisements for Happy Hive Preschool.
